NOTE:  Mercy Iowa City employee plan members only - The Mercy PHO has established an arrangement with Midlands Choice for Mercy employees with the Priority Health Network health plan to have access to providers in the Quad Cities who participate in Midlands Choice effective January 1, 2009.  The ID card will indicate Midlands Choice - Quad Cities Only and includes the following cities in the plan:  Bettendorf, Davenport, Moline, and Rock Island.  If you have any questions, please contact the PHO office at 319-339-3992 (press 1) or 1-800-637-2942 ext. 3992 (press 1).  Always check with the provider prior to making an appointment to make sure they participate with Midlands Choice.
