1. Is my provider in-network?
   You may locate in-network providers by clicking on the Priority Health Network link. You may also link to the Cedar Rapids PHO (www.cedarrapidspho.com) and Genesis HealthPlan (www.genesishealth.com/services/ghp.aspx) for providers who are in-network through agreements the PHO has with these two partners. The Mercy PHO makes no representations or warranties regarding the information contained on the Cedar Rapids PHO or Integrated Health Partners web sites, its suitability for use, or that the information will be error free. Always check with the provider prior to making an appointment that they participate in either one of these plans.
    

2. How do I verify my benefits?
   You will need to contact your employer's Human Resources department or the insurance company who administers or pays your benefits as each group's plan varies. The PHO is not an insurance company and does not process medical claims.
    

3. Does my plan require a referral?
   Please check your benefit book or your Human Resources Department to see if a referral is required for services according to your plan. Each employer who utilizes the Priority Health Network has different benefit levels and the Mercy PHO cannot direct you with regards to this question (with the exception below); therefore, it is necessary for you to check your health plan benefits.
